I tried it again with phpbbstafftest3, phpbbstafftest4 and phpbbstafftest5 using spread out times, only the last one arrived at that service. As I noted above you are probably hitting email quota limit which can explain inconsistencies.
First fix your captcha settings and use the Q&A, besides reducing emails sent it will stop a lot of spam registrants.
Disable the ability of user to send email through the board on the email settings page.
Next under Permission roles heading select user roles. Make sure each role has email functionality set to never, there is two of them. Repeat for forum roles, there is one permissions for this. Make sure you are using never.
This is what I would advise regardless of your issue. The captcha you are using has been broken for more than a decade and is useless. The email functionality opens you up to being spam relay.
