At this point there is really no difference except between images and the others. Images are served with image mime type so the browser displays it as image. Years ago the others would be served with appropriate mimetype so the users computer cold open it with the correct program(e.g. Word for .doc) but that was removed because of security concerns. Today they are all served using mimetype application/octet-stream which forces the browser to download file.

When adding extension just pick a group that seems appropriate. Under "Manage attachment extension groups" click the little green gear icon and make sure the group is allowed.

Something to be aware of, .doc, xls etc can contain viruses and checking for that is outside of the scope of phpBB security features. It shouldn't effect your forum but can certainly effect anyone downloading an infected file.
